| Description: |
| Legislative Authorities: Title V, Section 501a2 of the Social Security Act, as amended, 42 USC 701a2, the Secretary to provide for special projects for genetic disease testing, counseling, and information development and dissemination programs.
This program will fund two demonstration projects in two centers with national coverage that will link and work with public health clinics and CHCs. The centers will utilize staff to coordinate with Bureau of Primary Health Care funded and title V funded public health clinics for the dissemination and evaluation of impact of the use of previously developed family history and risk assessment tool. The centers will train the clinics involved in the project on the use of the tool and integration into regular clinic flow as part of the clinical history ascertainment. The centers will be responsible for measurement at the clinic level of baseline public health genetics and risk assessment practices and follow longitudinally at 6 month intervals for screening and clinical interventions based on family history and track changes over time. The centers will work with the clinics to follow a cohort of patients with increased risk for common disease based on family history and what impact that risk assessment has on health behaviors.
A clinical assessment of health behaviors will be measured over time based on pre and post education as indicated by the results of the risk assessment tool.
This project aligns with the HRSA strategic plan in the following areas: Goal I - Improve Access to Quality Health Care and Services, subgoal c Integrate primary care and public health by utilizing both public health and community health centers to link preventive services to prevention based on assessment of risk for common public health concerns. It also aligns with Goal III - Build healthy communities, subgoal c Strengthen the focus on illness prevention and health promotion across populations and communities by basing intervention and disease prevention on risk assessment and targeting interventions to those at highest risk. It also aligns with Principal 6 Focus on results across the population, by using the best available evidence, monitoring impact and adapting programs to improve outcomes, using genetic risk assessment from family history and evaluate the impact of this knowledge on preventing chronic disease.
