Sample Survey Questions to Explore the Internal and External Environments

Intake/Eligibility:

  1. Does your TANF automated information system (AIS) effectively capture all required data in or to processes applications and assist case managers complete individual service plans (such as new nonfinancial information related to work status, child care needs, English language proficiency, transportation services, housing needs, compliance with immigration requirements, fleeing felon status, domestic violence/mental health, dependents' school attendance, probation status, and substance abuse status)?
  2. Identify the internal agencies you depend upon to provide core intake and eligibility services. (i.e., county based governments or other groups within the HS Agency that  may accept TANF applications).
  3. Identify the external agencies you depend upon to provide core intake and eligibility services (such as, community-based organizations and nongovernment organizations which may accept applications or complete service plans).
  4. Identify the IT equipment the internal and external agencies depend upon to provide core intake and eligibility services.
  5. Does your TANF AIS generate an electronic authorization or referral for other services or programs (such as child care, transportation, employment services workshops, substance abuse programs, Food Stamps, Child Support and Enforcement (CSE), and Child Welfare, Medicaid/Children Health Insurance Program)?
  6. Is your intake/eligibility process part of a program-specific automation system that electronically processes applications to other integrated, automated critical functions within that program?
  7. Is your intake/eligibility function part of a centralized or decentralized automation system?

Case Management:

  1. Identify the internal agencies you depend upon to provide case management services.
  2. Identify the external agencies you depend upon to provide case management services.
  3. Identify the IT equipment the internal and external agencies you depend upon to provide core case management services.
  4. Does your TANF AIS effectively collect data and monitor case maintenance functions(such as monitoring that the individual is satisfactorily meeting service plan requirements, tracking when the individual is placed in a job, and tracking all critical date timelines and milestones that are critical to the continuation of services)?
  1.  Does your TANF AIS track and maintain Child Care Provider lists and license and compliance status changes?
  2. Is your case management function part of a centralized or decentralized automation function?

Federal Reporting:

  1. Identify the internal Agencies you depend upon to collect data to meet HS Agency's reporting requirements (Federal).
  2. Identify the external Agencies you depend upon to collect data to meet the HS Agency's reporting retirements.
  3. Does your TANF AIS collect the appropriate data in order to meet your HS Agency's reporting requirements (Federal)?

Financial:

  1. Identify the internal Agencies you depend upon to collect data to meet the HS Agency's financial requirements (State Treasury).
  2. Identify the external Agencies you depend upon to collect data to meet the HS Agency's financial retirements (financial institutions or EBT vendors).
