Revised form OFA-100, the Emergency Fund Request Form.  


Section 403(c) of the Social Security Act; TANF-ACF-PA-2009-01; TANF-ACF-PI-2009-05.  PURPOSE: To transmit the revised form OFA-100 and instructions for requesting grants from the Emergency Fund. 


The American Recovery and Reinvestment Act of 2009 (Recovery Act) established the Emergency Contingency Fund for State TANF Programs (Emergency Fund). It provides up to $5 billion to help States, Territories, and Tribes in fiscal year (FY) 2009 and FY 2010 that have an increase in assistance caseloads or in certain types of expenditures.  On July 20, 2009, we issued Program Instruction TANF-ACF-PI-2009-05 transmitting form OFA-100, the Emergency Fund Request Form, which the Office of Management and Budget (OMB) approved under its expedited clearance process. Approval for that initial version of the form has expired. After a period of public comment, we have made revisions to both the form and instructions to improve their clarity and help speed the process of awarding emergency funds.  Of particular note, we made the following changes: Added a table to Part 1 of the form for a jurisdiction to indicate whether it included administrative costs for any of the expenditure categories.  Provided a separate place in Part 2 for a jurisdiction to explain its method for estimating expenditures under each category. Added a table to the instructions in the “Application Dates” section to clarify when a jurisdiction may apply for emergency funds for a particular fiscal quarter.  Rephrased the wording in the same section to make it easier to understand when, due to expiration of the Emergency Fund on September 30, 2010, a jurisdiction must submit its final application and how we will use later revisions of data. Revised the instructions to Part 5 to remind jurisdictions to update the completion date and include a signed certification with each revision of form OFA-100. 


Effective immediately, a jurisdiction applying for emergency funds must use the revised form OFA-100. A jurisdiction should not resubmit any applications it made prior to the issuance of this guidance, but must use the revised form OFA-100 to update and revise data it has already submitted as part of an earlier application. 

We urge jurisdictions to review the revised form and instructions carefully to ensure that they submit accurate and complete Emergency Fund applications. This will help expedite review and approval of their funding requests.
