Family Support

Title:
"Texas Family Support Initiative"

Abstract:
This project proposes to begin an implementation process that builds on the work of the previous family support grant to maximize system change opportunities for families of children with disabilities. It will provide direction on policy for family support services and assume an additional planning and oversight role relating to implementation of key systems change proposals. Networks have been established and organized in order to plan local family support systems change strategies. Demonstration of the process and effects of implementing these system changes will be conducted in areas of the state with very different demographics, services, and family support systems. This will enable existing partnerships and various services organizations to to be strengthened, while at the same time expand relationships to other community partners and have results that show heightened awareness of family needs and perspectives.

"Missouri Partnership for Family Directed Supports"

"Supporting Elderly Families Caring for Sons and Daughters with Disabilities at Home"

Title:
"Increase Multi-Cultural Participation"

Abstract:
This proposal will focus on the sensitive needs for support services that address ethnic, cultural, and linguistic differences. It will use a past-experience approach of successes and lessons as a form of implementing services in the community. It will expand and broaden neighborhoods that are underserved and have not received available support and services. Plans are to connect and identify existing and newly created services. The project will use trained parents of children with disabilities and work closely with state agencies to help families become their own advocates.

"Virginia Is for Families"

Abstract:

"Partners in System Change: Building Family and Community Capacity"

"Delaware Family Support Project"

Abstract:
This project proposes to create a statewide Family Policy Council to support and improve collaboration across agencies and existing disability councils; provide major outreach, education, and technical assistance and training efforts to families and professionals across ethnic groups and generic services; and implement a pilot project designed to promote family control of resources. The project will target improving referrals and joint services to ensure use and quality of service availability for child care and social services to children with disabilities. Title:
"Stronger Voices: Extending the New Voices Initiative for Family Support"

Abstract:
The Governer's Council plans to develop an inter- agency approach for integrating family support and move state councils toward integration and inclusion in appointive leadership in all state functions. It will focus on development of customer on-line outreach and awareness, as well as information access services, which will include the creation of a public awareness campaign. Goals are to expand the information analysis data collections to include services in diverse communities and provide human resource development training and technical assistance for project participants and in- depth skills training for families of children with developmental disabilities. The generic community services network will be made up of participants with developmental disabilities from diverse communities.

Title:
"Family Leadership and Family Support System Change"

Abstract:
This project will address the need for systems change and continual development of a strong advocacy group of families and support of a statewide collaborative Task Force that provides guidance to these groups. There will be further development efforts to increase participation of minority families and aging family caregivers through outreach and educational activities. Specific goals are to conduct an ongoing assessment of policies and develop a strong family leadership initiative through training and technical assistance. "Implementing a Blueprint for Family Leadership and Professional Partnership"

Abstract:
Project objectives for this program are to increase the knowledge and skills of families with members who have disabilities, to enable them to participate fully and effectively in decisions related services. The program will encourage self-determination and advocacy and increase the knowledge and skills of families to impact policies and participate in governance and evaluation of systems of services and supports that assure choice and flexibility. Title:
"Double Jeopardy:"

Abstract:
This is a multi-agency collaboration to support older families who have sons and daughters with developmental disabilities. Plans for this project are to secure future services for persons with a disability as well as the aging parent. These agencies will jointly plan and implement this program at the state and local levels. Activities will include cross-training; training on estate planning for attorneys and other professionals; training of family members, including those with mr/dd; and outreach to and education of known and hidden families. The project will utilize lessons learned from Phase I and evaluate all training sessions and do a needs assessment/policy study on factors that affect family motivation. Products of this program will include a revised curriculum package, a media/promotional packet, and written reports of activities and materials that can be replicated nationally.

"Massachusetts Family Supports Model Demo Project "

This project proposes to build upon lessons learned during the past to expand capacity and promote systemic change. Plans are to develop a system that is flexible and family directed with the advisory guidance of a Family Support Group. Title:
"Kentucky Family Support Project"

Abstract:
Kentucky Spin proposes to expand family support activities statewide to include families and guardians of persons with disabilities of all ages, with a special focus on previously unserved and/or minority families. Through a coordinated system of all relevant state and local agencies, including Kentucky's school-based Family Resource and Youth Services Center, Special Education Parent Resource Centers, and family and consumer advocacy groups, this project will provide training to a minimum of 1,000 families on family supports, advocacy, and systems change. Participating families will be matched with a family who has successfully dealt with similar life issues (e.g., inclusive education, transition, supported or independent living, employment, etc.) or will be linked to other established support groups. Supports will be available on a regional basis, with more local, family-friendly centers for parent and families.

"Collaboration, Assessment, Planning, and Action, in our City"

Abstract:
This project proposes to further enhance and coordinate a statewide system to implement generic family support for families of children of all ages who have developmental disabilities. Through collaborative efforts with other city agencies, plans are to facilitate a system of reform through the development of Home and Communty Based Medicaid waiver options and other mechanisms to fund family support services. Efforts will be made to ensure support options to meet needs of older caregivers of adult children with developmental disabilities; to ensure a family-directed, community-based, culturally and linguistically competent system that makes use of person-centered planning and support; and promote legislation and policies that allow for services and supports that are flexible and reflect the mission and principles of the State Policy Council on Family Supports.

Title:
"PA Family Support Implementation and Coordination Project"

Abstract:
With collaborative efforts of state agencies and representatives of families of children with developmental disabilities, the goals of this project are to expand the current system of supports and create a comprehensive program for all citzens with DD, to include decision-making roles of individuals and family members, strengthen community capacity, and improve the quality of life for these individuals. The project will work to use state funds to provide respite services to children with physical disabilities along with existing state-funded family support programs; and with Council grantees and DD Act partners to create an array of coordinated and expanded supports that are cross disability, comprehensive, family directed, culturally competent, community based, coordinated across disability program offices, rooted in the use of generic resources and informal supports, and based on the concepts of learning and implementation of a self-determination model.

Title:
"Developing Statewide Family Support through Local Council Collaboratives"

Abstract:
The emphasis of this project is to establish a statewide system of Local Family Support Councils that is linked with existing local initiatives supporting families, to expand local participation of parents and family members of individuals with developmental disabilities, and develop community resources and natural supports. This project will disseminate and promulgate principles developed by the Family Support Council; set standards for models; and promote services statewide that are consistent, reliable, resource-based and person-centered, adapted to local community needs, and easy to find and easy to use. It further aims to collaborate with and build upon existing support networks for families that are not currently focused on DD, by providing training, technical assistance, and support around issues of disability, while continually working to integrate people with disabilities into those community networks and include local families in the development of policies, practices, and programs which directly impact their lives in the . community.

Title:
"Indiana Family Support Project"

Abstract:
The goals of this project are to develop a family support pilot project in the southeast region of Indiana to increase access to available family support resources by supporting the development of a statewide information and referral system. This project will be implemented by the Governor's appointed Family Support Council, consisting primarily of individuals and family members of individuals with disabilities. The aim of this project is to build on existing activities of support agencies involved in this project and train families to increase awareness of community supports to ensure that consumers are able to make informed choices. Dissemination of data collected from the support network will be transferred to the phone-based 211 referral system. This will increase the availability of community resources and educate individuals with disabilities on how to access community supports.

"The Connecticut Family Support Network"

Abstract:
This project proposes to build its Family Support Council and strengthen grassroots and local family support networks, as well as developing and implementing a statewide family support network, along with encouraging diverse participation in the support network. Project implementation will include training for all participants in the areas of best practices in family support, needs assessments, and the Study Circles Community Dialogues Series. The project will be made up of parents with children with disabilities and a parent coordinator will be identified for each regional area in Connecticut. Based on legislative and policy changes, needs assessment will be developed along with a work plan to address local needs. "DecisonMakers"

Abstract:
The Alabama Network of Support Councils, which is comprised of people with developmental disabilities and family members, will undertake efforts to implement this project with two primary objectives. Activities under the first objective will include public forums, a consumer satisfaction survey, needs reassessment, and a comprehensive review of family support policies and practices. To facilitate the review, the project will develop a self-determination-based framework for policy and practice analysis. The second objective is to create a strong and sustainable foundation for consumer-driven family support policy in the State. Activities will include the establishment of a state policy council, publishing a monthly newsletter with an emphasis on disability policy issues, developing an on-line home page for the program, a coordinated outreach effort targeting unserved and underserved people statewide, and a Family Support Best Practices Conference.
