Family Support 360:  Planning a One-Stop Center
A Project of National Significance from ADD

 

Title:
"Connecticut Family Support 360: A Planning Grant"

Abstract:
The purpose of this project is to plan multi-agency partnerships to design a one-stop center to assist poor or underserved families with a developmental disability, to preserve and strengthen and maintain the family unit. The project’s objectives includes the following; formalize partnerships; assess information technology; assess existing resources, conduct an analysis of eligibility across agencies/programs; assess training needs; conduct strategic planning; and develop an implementation plan. There will be three levels of formalized consumer involvement: 1) a committee comprised of persons with disabilities; 2) a committee comprised of statewide family and agency representatives; and 3) a consumer advisory board for the UCEDD.

Title:
"Developmental Disability Family Support One-Stop Center Planning Coalition"

Abstract:
This project will strengthen its support and service infrastructure by planning the implementation of a one-stop center for families with a member that has a developmental disability. Families will be targeted in the City of Wilmington which has a high percentage of families living in poverty and are of African American or Hispanic backgrounds that are poor. This plan will allow Delaware to develop a comprehensive and integrated support and service delivery system for individuals and families structured around holistic family centered services plan. The activities of this project will lead to fundamental alternations in outreach to targeted families, increase knowledge and information, access to services and a more comprehensive integrated approach to consumer and family support services.

Title:
"Iowa Family Support Initiative for Families and Children with a Disability"

Abstract:
This project will create a one-stop center to be used as a prototype to create additional program sites throughout the State. There will be two components to the one-stop model; 1) a technology-based information component and 2) individualized planning component. Implementing these two components will preserve, strengthen, and maintain families, while supporting families in obtaining better links to resources and services information, and technology advances that will help families connect with service providers. The Advisory group membership will consist of a Family Support Initiative Workgroup of at least three parents, state representatives, and a community based or faith-based organization. This group will contribute to the design of the center, along with a project manager that will assist with these efforts.

Title:
"Family Support 360 Project"

Abstract:
This project proposes a plan to facilitate the development of a family-driven one-stop support center in a predominately Hispanic neighborhood. This will be accomplished through the initial formation of a family advisory committee, which will be joined by representatives from collaborating local and state organizations. To help facilitate the development of the plan, a focus group will explore community needs, resources, and desired characteristics of a center. Feedback will be provided by Hispanic families on implementation of a cross-disability statewide needs assessment system, as well as legal and policy reports of issues for families in the community. Outreach activities and information will be provided through training and translation of existing resources materials. The advisory committee will finalize implementation plans for the one-stop center.

Title:
"First Stop"

Abstract:
This project will establish a planning council that will develop and implement a comprehensive multi-agency one-stop support center that will assist military families with a child or adult member with a disability. The planning council membership will consist of representatives of a diverse group of military families and primary stakeholders to determine the support needs of targeted families. The project objectives include outreach activities, training and technical assistance, and comprehensive family and community needs assessments. The needs assessments will involve open forums and comprehensive policy analysis that will guide the development of a vision, a mission, and a set of organization principles for implementation for the one-stop center.

Title:
"North Dakota One-Stop Project"

Abstract:
The project will establish a statewide partnership to develop an implementation plan for a one-stop center that will support families of children with developmental disabilities and concomitant mental health challenges. Objectives for this one-stop are to: 1) establish the partnerships needed to develop a plan, 2) gather and analyze the data necessary to implement the plan, 3) develop a statewide plan with appropriate input from families, partners, legislators, and state agencies, 4) build a consensus for implementing the plan, and 5) disseminate and communicate information about the plan.

Title:
"Ohio Family Support 360 Planning Grant"

Abstract:
The project will work with families who have children with developmental disabilities to plan a local single point of entry to create access to a one-stop center that is designed by a family group, family supported, family-centered, and family-directed. The family group will start by identifying obstacles they have experience and what will work for them. Also, the project will develop usable information in readily understood language, plan outreach activities, and circulate information. Surveys of disability information and the family group will identify information and referral needs. The family group will also review drafts of material for content and usability, and improve outreach and evaluations plans. Information technology will be explored to support the family group’s plan for a local single point of entry, based on the group’s plan.

Title:
"Family Support 360 Planning Grant for One-Stop Centers"

Abstract:
This project is working with multi-agency community partners to design a plan to implement a system for Tennessee’s one-stop centers that will provide family centered supports for poor and underserved families with a member who has a developmental disability. The community partnership will implement the planned system, develop a prototype web-base information system of existing database services and supports, and develop a prototype training program based on recommended practices. The project will begin by performing needs assessments for both the web-based information system and training package. Next, the prototype web-based information and training programs will be produced with consultation from the partners and pilot organizations.
