Title:
"Mountain View Family Support 360 One-Stop Center"

Abstract:
A plan of a Family Support 360 pilot project will be implemented to significantly improve access to comprehensive systems of family support services for families who are typically unserved and underserved. Core services of the pilot center will include; (1) family-centered planning, (2) information and referral, (3) education and training for parents and families, and (4) assistance with obtaining eligibility for other services. During the first year of operation, at least 50 families will be provided services. Outreach activities will also be conducted to assist additional families each year of the project.

Title:
"Colorado Family Support 360 Degrees (known as Colorado 360)"

Abstract:
This project will offer integrated services and supports that will improve access to programs and increase the capacity of the delivery of supports specific to the needs of families receiving TNAF who have a child with a developmental disability. Family support navigators will provide support to the targeted families and on-going comprehensive training and mentoring to TNAF case mangers. Other resources available through this project will include; assistance with difficult case reviews, parent support networks, advocacy with the public school system, advocacy for inclusive child care, and on-going follow along services to help families meet their desired outcomes.

Title:
"DC Family Empowerment Center"

Abstract:
Through a partnership with District service agencies and Project ACTION, a center will be created to provide one central location for DC families to access appropriate services and supports. The services of the Center will be family directed and culturally/linguistically competent. The project is committed to family governance and empowering families to control resources. Title:
"Navigational One-Stop System: Hawaii 360 Youth and Family Project"

Abstract:
In partnership with the State Developmental Disabilities Network and other State agencies, this project will implement a pilot of collaborative systems change for a Navigational One-Stop System (NOSS) in Hawaii for transitioning youth with developmental disabilities and their families. A community advisory committee will oversee the implementation of the project and ensure a streamlined service system for transitioning youth. Through the NOSS, targeted families will be identified, their needs assessed, Individual Family Plans developed, referrals conducted, and effectiveness of services evaluated.

Title:
"Idaho Family Support 360 Resource Center Project"

Abstract:
In collaboration with the Idaho Family Support Policy Council, this project will create an unduplicated, coordinated, comprehensive one-stop center pilot to support the expressed needs of families in a rural isolated region with family members who have developmental disabilities. The regional partnerships developed with the State’s Any Door Initiative Pilot Project, family support programs, and other support agencies will establish a network for the targeted families, that have an impact on systems throughout the State. This project will take place in the rural isolated region of the State of Idaho that incorporates Clearwater, Idaho, Latah, Lewis, and Nez Perce Counties.

"Massachusetts Family Support 360 Pilot: SCAN 360"

Title:
"One-Stop Family Support Resource Center in Baltimore City"

Abstract:
Through an inter-agency collaboration which cuts across the traditional agency lines for providing services, this project will implement a family-centered resource designed to preserve and strengthen families of children with intense needs. Maryland law describes children with intense needs as having “behavioral, educational, developmental, or mental health needs that cannot be met through available public resources. The Family Support Resource Center of Baltimore City will provide a comprehensive infrastructure enabling families to have a seamless, single point of entry with eligibility for and linkage to desired services. Specifically, the center will promote family leadership, enhance interagency collaboration, provide information and referrals, and offer family navigators to guide and empower families in obtaining what they need for their children and families.

Title:
"Detroit Family Support 360 Degrees Project"

Abstract:
The goal of this project is to preserve, strengthen, and maintain the family unit, while making sure that individuals with developmental disabilities receive ample opportunities to exercise greater choice and self-determination. Project activities will build upon the success of the planning grant for the one-stop center, which established a cohesive partnership of service providers and consumers. Through the implementation of a one-stop center, assistance will be provided to low-income, minority families in Detroit with a family member who has a developmental disability. Title:
"Visions with Hope 360 Degrees Center"

Abstract:
The project will establish the Visions with Hope 360 Degrees Center for providing disability services to Latino families. The center will be housed at the Mattie Rhodes Center, a non-profit agency serving Kansas City children, families, and community by providing social services, mental health counseling, and art experiences in a bilingual, culturally sensitive environment. The Visions with Hope 360 Degrees Center will provide three levels of support to families; (1) information and referral, (2) peer support, and (3) in-depth family planning. Bi-lingual staff will facilitate family-centered planning and connect families to different services. "Family Service Center (FSC)"

Abstract:
This project will implement the three one-stop centers planned to support the service integration needs for the Commonwealth of the Northern Marianna Islands (CNMI). The University of Hawaii’s Center on Disability Studies will provide on-going support to the project by sharing current theory, research, evaluation, and best practices related to family support and training. The languages spoken in CNMI include Chamorro, Carolinian, English, Japanese, Chinese, Filipino, Korean, and other Micronesian languages. More than half of the CMI population is non-resident alien workers who are primarily Filipino and Chinese.

Title:
"Family One-Stop"

Abstract:
In partnership with the Institute on Disability at the University of New Hampshire and the New Hampshire Developmental Disabilities Council, this project will build community bridges to improve career and recreational opportunities, financial independence, and family support options for individuals with developmental disabilities and their families in the 15 rural eastern Monadnock Region towns. The services of the One-Stop Center will focus on three areas: (1) Individual and family directed planning for community participation; (2) Transition planning and career development; and (3) Individual and family economic development. A minimum of 50 families will receive direct services annually as the project demonstrates improved community engagement and individual and family outcomes. "Family Support 360: Project Pueblo Connections Implementation"

In collaboration with the Five Sandoval Indian Pueblos, Inc., this project will implement a Family Support 360 Center in each of the five Pueblos to provide a comprehensive array of culturally appropriate services and supports for families who have a son or daughter with a developmental disability. "Oklahoma’s Family Support 360 Degrees Center"

Abstract:
The project will implement a one-stop center for families of low income who have children with developmental disabilities. The center will be located at the Pediatric Practice Model (PPM) Clinic within the Children’s Hospital on the Oklahoma University Health Sciences Center campus. At the center, Family Support Coordinators will guide families through the required paperwork for assistance from other agencies and ensure the families understand how to exactly receive the help they need. Title:
"Rhode Island Family Support 360"

Abstract:
This project is focusing on assisting two sets of targeted families; 1) families who have children with the most significant developmental disabilities in transition (13 to 21 years old) and are likely to be eligible for services from the Division of Developmental Disabilities and the Office of Rehabilitation Services; and 2) families in which the parents themselves have developmental disabilities. A one-stop transition center will be established to serve the first set of targeted families. "People Leading Accessible Networks of Support (PLANS)"

PLANS will pilot a one-stop center in rural northeastern South Dakota with the intent of providing a seamless, single point of entry service delivery model. Title:
"Green Mountain Family Support 360 Implementation"

Abstract:
This project will build a regional capacity to deliver comprehensive, flexible, coordinated supports for families (especially for parents with developmental disabilities) to strengthen the family’s ability to care for their children safely at home. Parents with developmental disabilities face numerous challenges, one of the most devastating being the loss custody of their children. Family Support 360 Technical Support:

In the fall of 2004, BETAH Associates of Bethesda, Maryland was awarded the contract to provide technical assistance to ADD’s Family Support 360 grantees. BETAH staff is coordinating the annual grantee technical assistance institute, hosting conference calls, providing expert knowledge upon request, and facilitating peer-to-peer grantee learning and problem solving. Additionally, BETAH is fostering collaboration between ADD’s Family Support 360 initiative and the Administration on Aging’s (AoA’s) and Center for Medicare and Medicaid’s (CMS’s) Aging and Disability Resource Centers. Through this contract, a technical assistance web site has also been developed which may be viewed at: www.addfamilysupport360.org.
