2009 Special Initiatives

Title: "Pathways MN Emergency Preparedness One-Stop Center"

Abstract:
This project will develop with its partners, a multi-agency Pathways Emergency Preparedness One-Stop Center in North Minneapolis that will provide to individuals with developmental disabilities and their families, the information and in-depth training they need to develop their own emergency preparedness plan, for a variety of emergency events such as H1N1 Pandemic, tornadoes and winter storms.  The main goal of the project is through the Center, individuals with developmental disabilities and their families will have the supports they need to remain intact and self-sufficient during an emergency event.

Title: "Hawaii Emergency Transition One-Stop"

Abstract:
This project will develop a plan that will create a one-stop center that will allow and prepare individuals with developmental disabilities and families with developmental disabilities to effectively prepare for and remain intact during emergency transitions.  The Hawaii Emergency Transition One-Stop project will review existing plans, work with current key stakeholders to revise where appropriate, and create a comprehensive plan that will outline the community strengths, needs, and approach to help individuals with a disability successfully respond to an emergency.

Title: "California Training and Technical Assistance Center for Emergency Preparedness and Developmental Disability "

Abstract:
This project will establish and coordinate the California Consortium on Emergency Preparedness and Developmental Disability to develop an implementation plan to create a one-stop center for individuals with developmental disabilities and their families to prepare and remain intact during an emergency transition.  This project’s goals are to, establish an online California Training and Technical Center for Emergency Preparedness and Developmental Disability, create coordinated responses and interagency efforts to prepare individuals with developmental disabilities and their families for emergency situations, develop an effective approach for providing individualized, emergency preparation support to 60 individuals with developmental disabilities.

Title: "Support All Families in Emergencies (S.A.F.E.)"

Abstract:
This project will develop an implementation plan for a one-stop center to coordinate shared resources and funds to train individuals with developmental disabilities and their family members to be prepared for possible emergency situations in Detroit, Wayne County, Michigan. The project will target participants that are unserved and underserved and at risk of being unnoticed in the event of a disaster. Partnerships will be forged between the disability community and emergency preparedness leaders, to ensure that when disaster strikes, citizens with disabilities have equal and responsive access to safety.

Title: "DE Family Support 360 Special Initiative"

Abstract:
The Center for Disabilities Studies (CDS) at the University of Delaware will conduct a one-year effort to advance the work that has already begun in two ways. First, they will build on the momentum of the existing planning group by delving more deeply into the issues, determining how best to invest resources in solutions the group has identified and advocating for law and policy changes to ensure that the need if individuals with developmental disabilities are addressed during emergency circumstances. CDS will also work in conjunction with neighboring jurisdictions (specifically, Maryland and the District of Columbia) to devise regional approaches to emergency planning and management that accommodate the needs of individuals with developmental disabilities.

Title: "One Stop Emergency Transition Center (OSETC)"

Abstract:
This project will establish a process to assist individuals with developmental disabilities and their families to develop Individualized Emergency Transition Plans (IETPs) to be used in the event of a natural or manmade emergency.  The project builds on the successful activities of the Strengthening Families One Stop Emergency Transition Planning project undertaken during the past five years.  The goals are to develop partnerships among individuals, families, resources, and service providers to assist individuals with developmental disabilities and their families to make emergency transition plans and to develop an implementation plan for the One Stop Emergency Transition Center.

Title: "Family Support 360: Project Pueblo Emergency Connections"

Abstract:
Pueblo Emergency Connections will assist underserved Native American individuals with developmental disabilities and their families in the Sandoval County Pueblos in New Mexico accessing services and supports, as well as prepare for emergency situations.  This project will be a collaborative venture between the Center for Development and Disability (CDD), Educating parents of Indian Children with Special Needs (EPICS), the Native American Independent Living Center, the Native American People First Chapter, the ARC of NM, tribal governance, as well as the state and county agencies for emergency preparedness.   The project will develop a coordinated and sustainable system of services and supports to Pueblo families who have a family member with a developmental disability.  Individuals with developmental disabilities and their families will also be trained to respond appropriately and safely in emergency situations.

Title: "Green Mountain Family Support 360 Special Initiative Emergency Planning Project "

Abstract:
This project will develop the Vermont 360 collaborative approach with peer navigators and navigator teams to support rural families with members with disabilities, both children and adults during emergencies.  Project partners will work together to develop specific tools and strategies to help families create Individual Emergency Preparedness plans, to prepare rural families who have family members with developmental disabilities to respond to the emergencies particular to the Vermont environment, and to develop materials and planning processes that will be available to the general public as well as the members of the disability community.

Title: "New Jersey 360 Outreach Life Safety Project"

Abstract:
The New Jersey Outreach Life Safety Project is focused on providing a minimum of 60 individuals with developmental disabilities and their families who reside in Middlesex County with a Life Safety Plan and Kits, 200 additional individuals with Go Bags, and assistance in participation in the New Jersey Register Ready program.  This project is built on a unique collaborative effort between CPAMC, consumers, its lead partner the NJ CDD, community service providers partner agencies, and the state and county OEMs who have all had input into the design of this project.  It will result in a replicable best practice model of service that can stand alone or tied to a 360 family Support Center.  It utilizes state of the art technology as well as one-to-one personal contact.  It also addresses the needs of a culturally diverse community.

Title: "Project 911"

Abstract:
The Center for Persons with Disabilities at Utah State University plan to use a participatory action model to design an implementation plan with an emphasis on emergency preparedness one-stop family support center (Family Support 360: Project 911) serving the targeted areas within Salt Lake County.  This model will serve at least 60 individuals with Developmental Disabilities (DD) or families of persons with DD.  A plan will be developed to effectively train individuals with DD and family members to prepare for emergency situations.

Title: "Family Support 360 Initiative: Emergency Transition"

Abstract:
This project will develop an implementation plan for at least one One-Stop Center in Franklin County, Ohio, that is based on input from individuals with developmental disabilities and family members, best practices and research on information technology so more people with developmental disabilities prepare for emergency transitions and remain intact with family during transitions.   This project plans to increase the knowledge of planning committee members in emergency transitions, emergency planning and methodology, and enhance decision making capabilities for planning committee members through research provided on best practices.

Title: "Family Support 360 Special Initiative: Emergency Transition for Individuals with Developmental Disabilities: Developing an Implementation Plan "

Abstract:
Quality Trust Family Empowerment Center defines an emergency as an occurrence of a natural catastrophe, technological accident, or human caused event that has resulted in severe property damage, deaths, and/or multiple injuries that puts the most vulnerable individuals at risk of bodily harm, loss of independence and destruction of their human support networks.  The project team will develop a variety of information, training and support services to promote and facilitate individual family and community emergency planning for District of Columbia residents and their family members with developmental and other disabilities.  This partnership result in the creation of a Mid Atlantic Family Support Resource Center on Emergency Transitions with collaboration and support for other and regional public and private agencies.
