About a third (834) of the cases were reviewed in 2001 and are not included in much of the case-level analysis, since there were changes in the rating criteria for a number of items after the 2001 reviews. The year-by-year breakdowns are:
2001: 322 IH + 512 FC = 834 total
2002: 325 IH + 423 FC = 748 total
2003: 279 IH + 361 FC = 640 total
2004: 166 IH + 181 FC = 347 total
