CFSP/APSR Toolkit: Tribal Requirements for FY 2009 APSR
Tribal Checklist for the Child and Family Service Plan (CFSP)/Annual Progress and Services Report (APSR) (PDF - 38 KB). This checklist was developed by Region 10 to help Tribes ensure their APSR submission includes all required elements. Items on the checklist have been shortened; see the Program Instruction for a complete description. The item's location in the Program Instruction is provided.
The APSR must include all of the following items. Missing or incomplete information will result in the withholding of funds for the programs affected, until such time as approval can be granted by the Administration for Children and Families.
The following items are requirements for completing and submitting an APSR:
- CFS-101 Parts I, II, III and Assurances - Information on the CFS-101, Parts I, II, and III.
- SF-269 Financial Report(s) for Annual Expenditures under Title IV-B Subparts 1 and 2 - Submission requirements for expenditures of title IV-B funds must be met by all grantees on the Financial Status Report SF-269.
- Service Description for Title IV-B Subpart 1 and/or Subpart 2 - Information on requirements that must be included in developing an APSR related to improving outcomes for children and families, expanding and strengtheing the range of existing services, revisions to existing goals and objectives under Title IV-B subpart 1, description of services to be provided in FY2008 under Title IV-B 1 and/or Title IV-B 2, reporting on population served, information on requirements related to coordination and collaboration efforts, including Healthy Marriage, Fatherhood, Youth Development, Rural, Faith-Based, and Community Initiatives and content and frequency of caseworker visits.
- Collaboration - Information on requirements related to ongoing activities of coordination and collaboration efforts and a descriptions of collboration efforts between the child welfare agency and the State or Tribal courts.
- Program Support - Information on how the Tribe's child welfare services staff will be developed and trained, technical assistance that the Tribes anticpating receiving, and discuss the management information and quality assurance systems that will be updated or implemented.
- State Consultation - Information regarding Tribal consultation with the State pertaining to the Indian Child Welfare Act (ICWA), description of who is responsible for providing protections for Tribal children and descriptions of how the Tribe has been consulted about the programs that will be carried out under the Chaffee Foster Care Independence Act.
- Consultations with Physicians or Appropriate Medical Professionals - Information on describing how the Tribe actively consults with and involves physicians.
If additional Title IV-B subpart 2 funds become available for FY 2008, in order to receive any additional funds, eligible Tribes need to submit the following:
