Iowa's comments to be included in Child Welfare Outcomes 2001: Report to Congress are the following:
Iowa's child welfare system serves children placed due to delinquency, emotional/behavioral problems, and abuse/neglect. Our caseloads and supervisor-to-worker ratios are two to three times national standards.
With regard to safety, child abuse reports have continued to increase. In 2001, Iowa's recurrence rate and incidence of abuse in care are both higher than the national standard. However, our 2002 data indicate an abuse in care rate of 0.40%, which is within the national standard. Last year we reviewed a sample of recurrence data, and found that 20% did not reflect subsequent abuse- rather, they reflected updated information on the same incident or an incident that occurred before our first contact with the family. Excluding these would reduce our recurrence rate to 9.4%.
With regard to permanency, Iowa's "time to reunification", placement stability, and "time to adoption" rates continue to meet the national standard. In 2002, DHS set an all-time record of 792 adoptions.
Iowa foster care re-entry rate is higher than the national standard. Re-entry rates are highest for youth age 11 and older who are in placement due to their behavior rather than abuse/neglect. In these situations, brief placements help to maintain youth in their permanent placement. Additionally, in Iowa when a child is returned home, the court returns custody to the family immediately rather than using a "trial home visit" where the state maintains custody for a period of time, allowing the child to come back into care without counting as a "re-entry".
Iowa's data indicates 18% of youth age 12 and under are placed in group care, which is higher than the national median of 3.4%. We will be examining this as part of our CFSR PIP.
DHS remains committed to working with the Courts and others to improve the safety, permanency and well-being of the children and families we serve.
The following discussion focuses on Iowa's performance with regard to each of the outcomes assessed in the Child Welfare Outcomes Annual Reports to Congress. The Iowa Child and Family Services Review (CFSR) was completed in fiscal year (FY) 2003, and compliance with the national standards was based on data from [year?]. The final report for that review is available from http://www.acf.hhs.gov/programs/cb/publications/cwo01. A discussion of the implications of CFSR findings for Iowa's performance on the Report to Congress Outcomes is provided in Child Welfare Outcomes 2000.
The State's performance is assessed by examining the percent change in the data for each outcome measure from 1999 to 2001. The percent change was calculated using data carried out to the second decimal point. However, with the exception of outcome measure 2.1, the data presented in the Report to Congress are rounded to one decimal for ease of reading the tables (measure 2.1 must be presented as two decimals because the national standard is less than one percent).
Outcome 1. Reduce recurrence of child abuse and/or
neglect
In calendar year (CY) 2001, the rate of maltreatment recurrence in
less than 6 months was 11.2 percent, which does not meet the
national standard of 6.1 percent or less and is higher than the
national median of 8.0 percent. Performance on this measure
declined from 1999 to 2001 (+33.2 percent change). The State notes
in its comment letter that this rate is inflated, because some
reports reflect updated information on the same incident or an
incident that occurred before the agency's first contact with the
family. Final Report information identified the following as
potential barriers to preventing maltreatment recurrence: (1) lack
of comprehensive assessments of risk to capture underlying problems
in the family, particularly mental health issues; (2) a scarcity of
substance abuse treatment services for parents; and (3) high
caseloads that make it difficult for workers to monitor
families.
Outcome 2. Reduce incidence of maltreatment in foster
care
In CY 2001, the incidence of maltreatment in foster care was 0.89
percent, which does not meet the national standard of 0.57 percent
or less and is higher than the national median of 0.47 percent.
Performance on this measure declined substantially from 1999 to
2001 (+98.5 percent change). CFSR Final Report information suggests
that performance on this measure may be attributed in part to: (1)
extremely high caseloads that make it difficult for workers to
monitor children in foster care; (2) placing too many children in
foster homes; and (3) insufficient matching of children's needs
with foster homes capable of meeting those needs.
Outcome 3. Increase permanency for children in foster
care
In FY 2001, 91.8 percent of children leaving foster care were
discharged to permanent homes (which is higher than the national
median of 85.3 percent); 87.2 percent of children with a diagnosed
disability who exited foster care were discharged to permanent
homes (which is higher than the national median of 78.4 percent);
and 88.0 percent of children exiting foster care who were older
than age 12 at the time of entry into foster care were discharged
to permanent homes (which is higher than the national median of
72.3 percent). In addition, 26.0 percent of the children who were
emancipated from foster care in FY 2001 were 12 years old or
younger at the time they entered foster care, which is less than
the national median of 28.0 percent. Although there were no
substantive changes in performance from FY 1999 to FY 2001 for any
of the measures related to the percent of children discharged to
permanent homes, there was a large decline in performance related
to the percent of the children emancipated from foster care who
were 12 years old or younger at the time they entered foster care
(+173.2 percent change). Information from the CFSR Final Report
identifies Family Team Meetings and Alternative Dispute Resolution
as measures contributing to permanency for children in foster
care.
Outcome 4. Reduce time to reunification without increasing
re-entry
In FY 2001, the percentage of all reunifications occurring within
12 months of children's entry into foster care was 81.1, which
exceeds the national standard of 76.2 percent and is greater than
the national median of 69.2 percent. Performance on this measure
did not change substantively from FY 1999 to FY 2001. CFSR Final
Report information identifies the following as supporting timely
reunification: (1) Family Team Meetings; (2) intensive services and
supports for families; and (3) frequent visitation between children
in foster care and their parents. However, the CFSR Final Report
also notes that the frequency of face-to-face contact between
caseworkers and children and parents is generally insufficient to
promote attainment of permanency goals.
Iowa's performance in FY 2001 with regard to the percentage of re-entries into foster care within 12 months of discharge from a prior foster care episode was 24.4 percent. This does not meet the national standard of 8.6 or less and is greater than the national median of 10.4. However, performance on this measure improved from FY 1999 to FY 2000 (-8.5 percent change). Information from the CFSR Final Report identifies the following as barriers to preventing re-entry into foster care: (1) premature reunification, and (2) lack of post-reunification supports. The State's self-assessment notes that the re-entry rate is also affected by a program to provide brief out-of-home placements to stabilize families in crisis.
Outcome 5. Reduce time in foster care to adoption
In FY 2001, the percentage of all adoptions finalized within 24
months of a child's entry into foster care was 46.0, which exceeds
the national standard of 32.0 percent or more and the national
median of 21.0 percent. Performance on this measure improved from
FY 1999 to FY 2001 (+23.0 percent change). Information from the
CFSR Final Report identifies the following as contributing to
timely adoptions: (1) timely filing for termination of parental
rights (TPR), and (2) use of cross-jurisdictional resources for
waiting children.
Outcome 6. Increase placement stability
In FY 2001, the percentage of children who were in foster care for
less than 12 months and experienced no more than 2 placements was
88.8, which meets the national standard of 86.7 percent or more and
exceeds the national median of 83.3 percent. Performance on this
measure did not change substantively from FY 1999 to FY 2000.
Information from the CFSR Final Report suggests that the
pre-service foster parent training using the Parenting for Safety
Model Approach to Partnerships in Parenting (PS-MAPP) curriculum
contributes to placement stability.
Outcome 7. Reduce placements of young children in group homes
and institutions
In FY 2001, the percentage of children age 12 or younger placed in
a group home or institution was 22.6, which is higher than the
national median of 8.7 percent. Iowa's performance on this measure
improved slightly from FY 1999 to FY 2001 (-5.0 percent).
Information from the CFSR Final Report notes that children are
often placed in emergency shelter care by law enforcement and that
length of stay in shelters is increasing due to lack of foster
homes and a long waiting list for treatment facilities.
