placement, care, or supervision (45 CFR 1355.40(a)(2)).

section 422(b)(10) of the Social Security Act (the Act) (Appendix A
- SECTION II).

Make any necessary modifications to FACTS to
allow the entry of information on children in the agency's care,
placement, or supervision from the Division of Child Mental
Health.Revise the program code to identify and extract the CMH
children in the State agency's care, placement, or
supervision.Implement training of workers on the entry of the
appropriate information for these children. |

whether the child is returned home while still under the agency's
care, placement or supervision.
Modify FACTS to allow the entry of a discharge date, and reason,
if the child was returned home for a non-specified period of time
that exceeds six-months.

information for these children.
