ACTION TRANSMITTAL

TO: STATE AGENCIES ADMINISTERING CHILD SUPPORT ENFORCEMENT PLANS UNDER TITLE IV-D OF THE SOCIAL SECURITY ACT AND OTHER INTERESTED INDIVIDUALS

SUBJECT: Re-issuing the Locate Data Sheet

PURPOSE: As noted in PIQ-01-03, we realized after the distribution of the revised Federally approved Uniform Interstate Family Support Act forms that one of those forms, the Locate Data Sheet, incorrectly included reference to non-IV-D cases on the form and on its instructions. With this Action Transmittal we are re-issuing the form with that reference and the reference in the instructions deleted.

As we did in PIQ-01-03, we want to re-emphasize that the only purpose of the Locate Data Sheet is to permit one State to transmit to another State a request for assistance in locating a parent or the noncustodial parent’s income or assets for child support purposes in a IV-D case.

We will be making this revised form available on our web site as soon as possible.
