Standard Interstate Forms

TO ALL STATE IV-D DIRECTORS

Dear Colleague:

Since implementation of standard interstate forms, users have provided the Office of Child Support Enforcement with much positive feedback regarding the forms. For example, comments indicate that they provide uniformity, are more legible, and actions requested by other States are clearer.

In addition to positive feedback, we also received many constructive suggestions. To respond to these suggestions, we established the Standard Interstate Forms Committee whose members represent the many entities involved in interstate case processing. The enclosed photocopies of revised interstate forms are a result of the Committee's work. While they are not to be used until January 1, 1991 we are sharing an advance copy with you since many States have requested advance information which will assist in planning automated system changes.

The major changes in the revised forms are the following:

-The Uniform Reciprocal Enforcement of Support Act (URESA) Action Request and the Interstate Child Support Enforcement Transmittal forms have been combined into one form, now called the Child Support Enforcement Transmittal.

-Since many judges prefer to write their own orders, the Order and Judgment form has been eliminated. Instead, the court order will be attached to a new form, the Order Transmittal. This form provides a standardized vehicle by which the responding jurisdiction can transmit pertinent information contained in the court order.

-The General Testimony for URESA has been reduced from 12 to 8 pages. Language has been added to clarify which portions of the form can be skipped as a result of specific case circumstances.

-To facilitate location, a Locate Data Sheet has been designed. This form provides a standard format for providing information regarding the absent parent.

-For clarification purposes, the Paternity Affidavit has been revised considerably.

The Office of Management and Budget has approved the revised forms, but we still must complete minor tasks. (You may note that instructions for completion of the forms are not included.)

We anticipate all work to be completed by late June or early July. At that time, we will provide you with a camera-ready set of forms. Moreover, a new feature will accompany the camera-ready set -- diskettes which provide the capability to generate the forms on IBM PC-compatible systems without the use of a commercial software package.

Finally, States have discretion in deciding how to produce the forms, (printing, photocopying, or via computer generation). As always, computer-generated replicas must be in the same format and contain the same information as the forms.
