Data Entry Form OCSE-4

TO STATE IV-D DIRECTORS

For many years, the Office of Child Support Enforcement (OCSE) has provided State Child Support Agencies with the OCSE-4--

"Request For Federal Parent Locator Service". The OCSE-4 is a concise data entry form which contains all the fields necessary to submit cases. Throughout the years, Child Support offices have become more automated and are submitting cases directly on line to the Federal Parent Locator Services (FPLS). Other States have incorporated FPLS requests into their SPLS locate forms, thereby decreasing the need for a manual data entry form. As a result, very few States request or use the OCSE-4.

Due to budgetary restraints, we have been notified that OCSE will no longer be able to provide States Child Support offices with printed copies for the OCSE-4. We will continue to provide OCSE-4's only to Guam and the Virgin Islands as we do not provided on line access to these jurisdictions. If your office still uses the OCSE-4, you may consider reproducing the form though conventional copying methods or having it professionally printed.
