Standard interstate forms

TO STATE IV-D DIRECTORS

Dear Colleague:

Standard interstate forms first were developed in 1986 to respond to the need for uniformity and timeliness in establishing and enforcing paternity and child support obligations in interstate cases. Since implementation of the forms, the Office of Child Support Enforcement has received positive feedback and constructive suggestions regarding the forms. To respond to the suggestions, we established the Standard Interstate Forms Committee whose members represent the many entities involved in interstate case procesing. The revised interstate forms are a result of the Committee's work.

We also have developed, through the Child Support Technology Transfer Project, a diskette which provides the capability to generate the interstate forms on personal computers that operate in a MS DOS environment.

As stated in the "Dear Colleague" letter dated August 7, 1990, effective immediately you have discretion in using either the original or revised forms. To ensure conformity of form usage, initiating cases commencing January 1, 1991.

The method by which State and local agencies produce multiple copies of the forms is left to their discretion. Some examples include carbon packs and photocopies. As is the present policy, the State IV-D agency is responsible for printing and distributing the forms to appropriate IV-D staff. Enclosed in this mailing is a camera-ready set of forms and a diskette with accompanying instructions. NOTE: PLEASE KEEP THEM IN A SAFE STORAGE AREA WHEN NOT IN USE. WE ARE PROVIDING EACH STATE IV-D AGENCY WITH ONE CAMERA-READY SET OF FORMS AND ONE DISKETTE.

Specifically, the mailing includes the following:

-FORMS: For users who wish to continue completing the interstate forms by typewriter, we have enclosed camera-ready copies of the forms for printing.

-FORM INSTRUCTIONS: States are free to determine where they will print the instructions. Certainly, the instructions must be readily available to form users. We have enclosed camera-ready copies of the instructions for use by States that wish to print the instructions on the back of the forms. We also have enclosed the instructions in photocopy form for those States that wish to maintain them in manuals. Unlike the camera-ready set of instructions, the photocopy set has an introductory page titled "Interstate Forms Package - General In structions".

-PRINTING SPECIFICATIONS FOR STANDARDIZED INTERSTAE CHILD SUPPORT FORMS: We have enclosed printing specifications for each form. States that choose to have carbon packs of the forms will need to share specifications with the printer.

-DISKETTE FOR CSEFORMS VERSION 1.01: The enclosed 5 1/4" double density diskette provides the capability to generate the interstate forms on personal computers that operate in an MS DOS environmet. (Please read the accompanying instructions before installing the program.) You may make multiple copies of the diskette and distribute them to whomever needs them.

-INSTRUCTIONS FOR CSEFORMS VERSION 1.01: It is extremely important that you read this document prior to loading the diskette. It provides information on required equipment, installation procedures, and the functions and utilities of the program. (The structure of the database file is available upon request.)
