Forms OCSE-56 and 156

TO ALL STATE IV-D DIRECTORS

Dear Colleague:

As you know, we have revised form OCSE-56 with the assistance of the IV-D Directors' Reporting Committee which was chaired by Tony DiNallo. Due to the additional data collection requirements of the Family Support Act of 1988 (P.L. 100-485), it has been necessary to divide the OCSE-56 into two forms-- the OCSE-156 (Child Support Enforcement Program Annual Data Summary Report). We have previously distributed drafts of the new forms and instructions and would like to thank you for your comments. We have tried to address as many of your concerns and incorporate as many of your suggestions in the new set of instructions as was feasible.

Enclosed please find an advanced copy of the action transmittal, a set of new instructions with attachments, and several copies of OCSE-156 and OCSE-158. The new OCSE-156 must be used with the report submitted for the first quarter of fiscal 1991 (due January 30, 1991).
