TO: State and Tribal Agencies Administering Child Support Enforcement Plans under Title IV-D of the Social Security Act and Other Interested Individuals
RE: Standard Interstate Child Support Enforcement Forms - Opportunity to Provide Input Prior to Official OMB Clearance Process
Dear Colleague:
On December 18, 2000, OCSE distributed revised standard interstate child support enforcement forms via OCSE-AT-00-11. Those forms and related instructions are approved by the Office of Management and Budget for use through December 31, 2003.
In the next few weeks, OCSE needs to revise the forms, if necessary, and resubmit them for approval. Before initiating the official OMB clearance process, we are offering interested parties an opportunity to let us know of any changes that may be needed on these forms or on the instructions to the forms. We do not anticipate that major revisions will be required, but want to ensure that we identify any changes that would better address the needs of those who use these forms. The forms are available on the OCSE web site at http://www.acf.hhs.gov/programs/cse/forms/ under the heading UIFSA Forms. The forms in the standard interstate forms package are:

Transmittal 2 – Subsequent Actions
Transmittal 3 – Request for Assistance/Discovery
Notice of Determination of Controlling Order
Locate Data Sheet
Affidavit in Support of Establishing Paternity

General Testimony
Other OCSE forms (e.g., notice of lien, administrative subpoena, order/notice to withhold income for child support, and national medical support notice) are on separate clearance schedules, so we are not seeking comments on those forms at this time.
If you wish to offer input to OCSE in writing, we would appreciate receiving your comments by September 12. Comments may be sent either:
• By mail to the Office of Child Support Enforcement, Administration for Children and Families, 370 L’Enfant Promenade, SW, 4th Floor, Washington, DC 20447, Attention: Division of Policy, or
• By e-mail to ocseformscomments@acf.hhs.gov
We are also hosting a workshop at the 13th National Child Support Enforcement Training Conference on September 8-10, 2003, entitled “Updating the Federal Interstate Forms.” This workshop will feature a constructive discussion of the standard interstate forms and seek input on using them effectively and efficiently. If you are coming to this conference, please consider attending this session.
In addition to these informal ways to comment on the forms, there will be a 60-day period for official public comment, when we publish proposed forms in the Federal Register.
