ATTACHMENT: Annual Certification Form
DATE: July 22, 2005
TO: ALL STATE IV-D DIRECTORS
RE: Expanded Federal Parent Locator Service Safeguarding Certification
Dear Colleague:
The Federal Parent Locator Service (FPLS) annually requires all states to submit certifications of compliance with safeguard and security requirements relating to locate information received from the FPLS.
Please complete the certification form and return it to the FPLS by September 30, 2005. This will cover the period of October 1, 2005 through September 30, 2006.
Enclosed please find a sample of the certification form. Sample "A" is an example of the annual certification required for all states and local jurisdictions with access. All forms should be on state letterhead with the signature of the State IV-D Director or designee.
Please immediately fax a copy to Joan O’Connor at (202) 401-5558, and mail the original for our records to the following address:
