Please fax this information to Lamonda Fraley at (202) 401-5553, or email to scollections@acf.hhs.gov no later than August 15, 2001.

STATE: _______________

  1. Is your State sending an Annual Submittal File?
  2. If the Federal Office of Child Support Enforcement (OCSE) issues your Pre-Offset notices, do you want a Pre-Offset notice sent for all of your cases?
  3. The remaining questions apply only to those States that are sending an Annual Submittal File:

  4. Please provide the approximate number of cases that will be in your Annual Submittal File.
  5. Based on the schedule provided in the enclosures, please provide the approximate date your Annual Submittal File will be transmitted to OCSE.

The timeframe for your call will be based upon which hold period you use. The dates and times for the calls will be issued at a later date. Please indicate if your technical support team is interested in participating.

If this information changes, please notify the Federal Collections Branch as soon as possible.

IMPORTANT NOTE: States sending an Annual Submittal File must contact the Federal Collections Branch at (202) 401-9389 before transmitting.
