Insurers and third-party processors must complete the Debt Inquiry Service (DIS) Agreement and Profile form to register and participate in Debt Inquiry Service.

The Debt Inquiry Service allows insurers to report upcoming claimant payments to child support agencies in an easy and efficient way. Individual inquiries can be made or files may be uploaded to the application where they are matched to individuals who owe child support. Matches are reported to the child support agencies in participating states. Agencies then contact the insurer to withhold from the payment.
