Forms

Employers must provide information on employees, including financial information on wages, withholdings, and health insurance to child support agencies upon request.  This information is used to establish and enforce child support orders. While each state child support agency sends its own verification of employment (VOE) form, employers may respond using the standard verification of employment (VOE) response form, if the state child support agency accepts the form.  This standardized response form provides greater efficiency for employers.
