Dear Colleague:

Attached is the final version of the standard Verification of Employment (VOE) response form. As described in DCL-10-19, states will continue to send their state-specific VOE forms and employers will return the standard VOE response form to those states agreeing to accept it. The implementation date was January 1, 2011. A table identifying states that responded to date regarding their acceptance of the form is attached.

Comments submitted by states requesting changes to the standard response form are being collected for possible future revisions; however, such changes will not be included in the standard VOE response form at this time.
