Definition

The total monthly dollar amount (rounded to the nearest dollar) paid or to be paid to the provider for the care of the child. The total monthly amount should include Federal, State, and locally funded amounts.

Guidance

For each child receiving care, report the total monthly dollar amount (round to the nearest dollar and use leading zeros as necessary) paid or expected to be paid to the provider for the care of the child that occurred during the report month. The Total Monthly Amount should include Federal, State, and locally funded amounts. It should not include the family’s co-payment. Note that a zero is used for a specific reason; for “unknown/not reported” use a blank or space.
