Guidance: The dollar amount is rounded to the nearest dollar using leading zeros as necessary. If, on a case-by-case basis, income is not used to determine eligibility for protective service, this does not have to be reported. Except for unusual circumstances, if Element 9 is greater than zero, then Element 10 should be 1 (Yes). If Element 9 (Total Monthly Income for Determining Eligibility) is “unknown/not reported” do not use zero fill, use a blank or space.
