Exhibit 3-2 demonstrates both the array of places and types of organizations that facilitate application for SCHIP benefits. This includes call centers, CBOs, Welfare/Medicaid Offices, schools, mail, hospitals, health plans, health fairs, and health clinics.

Exhibit 3-3: A Typical TANF/FSP/Medicaid Application Process

Exhibit 3-3 provides a generic descriptive image of the steps of the TANF/FSP/Medicaid application process. The steps that lead up to eligibility determination begin with reception, and lead to screening, up- front requirements, and eligibility interview. Families applying for TANF/FSP/Medicaid through traditional means, namely the welfare office, most commonly experience these steps.

Exhibit 3-5: Sample TAN F and Food Stamp-Only Application Process (Study Sites)

Exhibit 3-5 outlines the steps involved in the application process for families applying for TANF/FSP/Medicaid benefits in each of our sites as they were as of June – December 2001.

In Dallas, TX the steps leading to eligibility determination begin with reception and move to eligibility interview, workforce orientation, finger imaging and photographing.

In Sedalia, MO the process begins with reception/screening, and moves to eligibility interview, orientation, work registration, and job search.

In New York NY, the process begins with reception, and moves to the eligibility interview. At this point the process can progress to finger imaging and photographing, eligibility verification review, or job search. From finger imaging and photographing the process then advances to eligibility determination. From eligibility verification review, the process advances to a home visit and then eligibility determination. From the job search, the process advances to job search classes, and finally eligibility determination.

In Seattle, WA the steps leading to eligibility determination include reception/screening, and an eligibility interview.

In Arlington, VA the steps leading to eligibility determination begin with reception, and progress to screening, orientation/application preparation, and an eligibility interview.

In Raleigh, NC the steps leading to eligibility determination begin with reception, and then progress to screening, an eligibility interview, and work registration.
