SUBJECT: Renewal of Information Collection Forms ORR-3 and ORR-4 for Unaccompanied Refugee Minor (URM) Program Reporting
The purpose of this State Letter is to announce to State Refugee Programs and Voluntary Agencies the new version of Forms ORR-3 and ORR-4 approved by the Office of Management and Budget (OMB) on April 20, 2010. Changes to the information collection forms are effective on that date.
Forms ORR-3 and ORR-4 (OMB No. 0970-0034) were designed to provide the Director of the Office of Refugee Resettlement (ORR) information on all children in the Unaccompanied Refugee Minor (URM) program, pursuant to Section 412 (d)(2)(B)(iv) of the INA (8 U.S.C. 1522). ORR currently requires grantees to submit two reports: an Unaccompanied Refugee Minor Placement Report (Form ORR-3) which is submitted upon initial placement of the child in the State and whenever there is a change in the child’s status, including termination of the program, and an Unaccompanied Refugee Minor Progress Report (Form ORR-4), which is required annually to indicate the child’s progress toward established goals in the case plan and the agency’s efforts toward family reunification.
Since publication of the regulations at 45 CFR Part 400 Subpart H there have been significant changes in the program through ORR-issued policy guidance and new Federal legislation. These changes have required ORR to revise both the ORR-3 and ORR-4 to collect accurate and useful information on children in the URM program.
The requested revisions to the ORR-3 and ORR-4 forms are designed to incorporate changes in the operation of the URM program, including the presence of newly eligible populations of minors. These revisions include: revising form titles to reflect the program name, Unaccompanied Refugee Minor (URM), and reflecting the use of Form ORR-4 as an “Outcomes” vs. “Progress” report; additional categories of placement origin and eligibility type; additional options of placement type and child welfare services (including independent living); request for placement cost; request for medical coverage information; extended educational, vocational and social adjustment service provision with outcome measures similar to National Youth in Transition Database (NYTD).
