Model Agreement between the Defense Manpower Data Center, Department of Defense (DOD); Administration for Children and Families, Department of Health and Human Services (HHS); and individual State Public Assistance Agencies (SPAAs).

The signatory to this Model Agreement authorizes the entity named below to participate in the Public Assistance Reporting Information System (PARIS) project activities described herein. The PARIS project will match public assistance data against data from The Department of Defense as described in the Computer Matching Agreement executed by the DOD Data Integrity Board on January 24, 2008 and most recently approved by the HHS Data Integrity Board on February 29, 2008 and published in the Federal Register dated April 30, 2008. The entity named below acknowledges its understanding of the terms and conditions of said Computer Matching Agreement. The entity named below further acknowledges that its compliance with the terms and conditions of the Computer Matching Agreement is required in order to participate in the PARIS project and, accordingly, it agrees herewith to abide by those terms and conditions.
The signatory to this Model Agreement hereby affirms that he or she is authorized
to enter into this agreement on behalf of the entity named below.
