Child Welfare
Title IV-B Reporting Requirements, Parts 1 and 2



Report What is it? For period Due date
Child and Family Service Plan (CFSP or 5 Year Plan) A narrative plan, which describes your child welfare vision, goals, and objectives (steps to meet your goals). FY 2005-2009 (next) June 30, 2004 (or June 30th prior to year for which the Tribe first applies for IV-B)

(Due every 5 years)
Annual Progress and Services Report (APSR)
  • A narrative report on progress made towards meeting each goal and objective in the past year;
  • A report on any changes in your goals and objectives; and
  • A narrative description of the services to be provided in upcoming year.
October 1 - September 30
(Federal Fiscal Year)
June 30, 2005 (or June 30th prior to year for which Tribe is applying)

(Due every year following CFSP)
CFS-101 Annual Budget Request,
Part I and Part II
A 2-page fiscal form used to request Title IV-B funds, and to show projected expenditures. (submitted with the CFSP or APSR) October 1 - September 30
(Federal Fiscal Year)
June 30th prior to year for which Tribe is applying

SF-269 [PDF, 147 KB] A fiscal form used to report Title IV-B expenditures. October 1 - September 30

Due no later than December 31st, after the end of the grant period.
