These are the federal forms that state child support programs are required to use in interstate cases, pursuant to section 454(9)(E) of the Social Security Act.
Administrative Subpoena: This form is to be administratively issued by the IV-D program to subpoena financial or other information needed to establish, modify, or enforce a child support order. A state may also elect to use this form in intrastate cases.
Notice of Lien: This form is used to assert liens on assets discovered through the Financial Institution Data Match process. The IV-D program may use the form to impose liens in intrastate cases also. For non-IV-D orders, an obligee or private attorney also could use this form.
