SF-424 Mandatory Form

In accordance with policies for mandatory grant programs, state child support agencies must submit the SF-424 Mandatory Form  (PDF) through the On-Line Data Collection (OLDC) system.  The SF-424 Mandatory Form captures important grant information, such as the Unique Entity Identifier (UEI) and Data Universal Numbering System (DUNS) number.  States may submit the SF-424 Mandatory Form when other required reports are due in the OLDC system, such as the OCSE-34A, 396A, or 157.
