SUBJECT: Standard Intergovernmental Child Support Enforcement Forms — Expire Feb. 28, 2026

ATTACHMENT: Intergovernmental Child Support Enforcement Forms 

The Office of Management and Budget approved the 13 Intergovernmental Child Support Enforcement forms and instructions (OMB No. 0970-0085) without change for use in intergovernmental case processing. The new expiration date is Feb. 28, 2026. 
The intergovernmental forms include:

  • Child Support Agency Confidential Information Form
  • Child Support Locate Request
  • Declaration in Support of Establishing Parentage
  • General Testimony and instructions
  • Letter of Transmittal Requesting Registration
  • Notice of Determination of Controlling Order
  • Personal Information Form for UIFSA 311
  • Child Support Agency Request for Change of Support Payment Location Pursuant to UIFSA § 319
  • Child Support Enforcement Transmittal #1 — Initial Request
  • Child Support Enforcement Transmittal #1 — Acknowledgment
  • Child Support Enforcement Transmittal #2 — Subsequent Actions
  • Child Support Enforcement Transmittal #3 — Request for Assistance/Discovery
  • Uniform Support Petition

The notice for public comment on the forms was posted to the Federal Register on March 28, 2022, in Volume 87, Number 59, Page 17297  (PDF).

ACTION REQUIRED: Use these forms with the Feb. 28, 2026, expiration date for interstate case processing.
