SUBJECT: Extension of OCSE-34 and OCSE-396 – Expire December 31, 2020

OCSE-396 Form and Instructions

The Office of Management and Budget (OMB) has granted an extension of the following forms with OMB control number 0970-0181 and an expiration date of December 31, 2020:

  • Form OCSE-34 – used by states and tribes to report collections, distributions, and disbursements
  • Form OCSE-396 – used by states to report expenditures and estimates

These versions include minor changes:

  • The submission due date for each form was changed from 30 days to 45 days following the end of each quarter.
  • Line 6 on Form OCSE-396 has been deleted and the instructions amended accordingly.

ACTION REQUIRED: Use these forms beginning in FY 2018, for the quarter ending 12/31/2017 and thereafter. Since the nature of the changes are minimal, any reports already submitted for that quarter need not be resubmitted.

SUPERSEDES: OGM-AT-15-01, OGM-AT-14-01, OCSE-AT-14-14

EFFECTIVE DATE: These forms are effective beginning FY 2018.
