OMB-approved Hague Child Support Convention Forms

SUBJECT: OMB-approved Hague Child Support Convention Forms

The 14 Hague Child Support Convention forms have been approved by the Office of Management and Budget (OMB Number 0970-0488) and are available for use by state child support agencies in international cases processed under the 2007 Hague Child Support Convention.

The Hague Convention forms include the Transmittal form, the Acknowledgment form, and 12 case processing forms grouped together by type of application. Now that the Convention is in effect in the United States, child support agencies must use the Transmittal for outgoing cases to Convention countries and the Acknowledgment for incoming cases from Convention countries. State agencies will receive all incoming Hague applications from Convention countries on the Hague forms.

When IV-D child support agencies send applications to Hague countries under the Convention, they will likely use the Hague case processing forms; however, child support agencies should always check the form requirements of the responding country before sending any request. Form requirements for each country are available on the specific Country Profile available on the Hague Conference website.

ACTION REQUIRED: State agencies are required to use the Hague Convention forms for case processing with Convention countries, according to the document requirements of the Hague Convention and other Convention countries.

EXPIRATION DATE: The new expiration date for the Hague Child Support Convention forms is April 30, 2020.
