NDNH Research Request Form

OCSS may disclose certain information contained in the NDNH to a requesting federal, state, or local agency for research likely to contribute to achieving the purposes of part A or part D of title IV of the Social Security Act. The disclosed information cannot include names, Social Security numbers, or other personally identifiable information.

To initiate a request for disclosure of NDNH information for research, the federal, state, or local agency must submit this request form (PDF) to OCSS to begin the review process.
