PURPOSE
To inform states and U.S. territories (hereinafter referred to as Community Services Block Grant (CSBG) grant recipients) of the requirement to revise and resubmit their CSBG Eligible Entity List.
BACKGROUND AND CURRENT STATUS
As discussed in CSBG-DCL-2022-39, the Office of Community Services (OCS) updated the CSBG Eligible Entity List to include three new fields:
- Executive Director (or the comparable position)
- Website Address
- Unique Entity Identifier (UEI), which replaced the DUNS number as of April 4, 2022
The Executive Director and Website Address were both added to ensure that OCS has the most up-to-date contact information for each CSBG-eligible entity.
Within CSBG-DCL-2022-39, OCS described the “longer term next steps”, including:
- Office of Management and Budget (OMB) Non-substantive Change Clearance: OCS submitted a non-substantive change request to OMB to include the UEI. COMPLETE
- Revisions in GrantSolutions.gov: Updating the CSBG Eligible Entity List within GrantSolutions.gov to include the three new fields. COMPLETE
- Revisions to SmartForms, Databases and XMLs: OCS will work with the National Association for State Community Services Programs (NASCSP) to update all necessary forms in preparation for the Federal Fiscal Year (FFY) 2024 CSBG State Plan and Fiscal Year (FY) 2023 CSBG Annual Report. PARTIALLY COMPLETE
- SmartForms have been updated. For OCS to complete testing, CSBG grant recipients must complete their revisions to the CSBG Eligible Entity List.
- Once testing is complete, OCS will meet with states that use databases for any subsequent updates to their state databases to align with the change to the UEI.
SUBMISSION AND REQUIREMENTS
All CSBG grant recipients must revise and resubmit their CSBG Eligible Entity List by July 14, 2023, and before initializing their FFY 2024 CSBG State Plan (as applicable).

- UEI #: Required narrative field; new field to be completed by all
- DUNS #: Read-only field; this field is being maintained for historical purposes
- CSBG Eligible Entity Official Name: Required narrative field; already completed
- CSBG Eligible Entity DBA: Optional narrative field; already completed
- CSBG Eligible Entity Executive Director: Required narrative field; new field to be completed by all
- Street Address: Required narrative field; already completed
- City: Required narrative field; already completed
- State: Required narrative field; already completed
- Zip code: Required narrative field; already completed
- CSBG Eligible Entity Website Address: Required narrative field; new field to be completed by all
- Geographical Area: Required narrative field; already competed
- Public or Non-profit: Dropdown field; already completed
- Type of Entity: Dropdown field; already completed
All CSBG grant recipients must also review the CSBG Eligible Entity List and add or remove CSBG eligible entities as applicable.
TRAINING, TECHNICAL ASSISTANCE AND SUPPORT:
To assist with the CSBG Eligible Entity List submission, OCS created the CSBG State Plan Toolkit. The CSBG State Plan toolkit is includes the most recent communications, guidance (OLDC step-by-step guides and manuals), the current version of Office of Management and Budget (OMB) approved forms, fillable forms, webinars, and training demos.
