Dear Colleagues,

The purpose of this message is to provide you the survey and links to the accompanying documents that the Office of Community Services (OCS) is using to gather feedback regarding the implementation of the Low Income Household Water Assistance Program (LIHWAP). The survey must be completed no later than Tuesday, April 27, 2021. If you complete the survey after that date, we cannot guarantee that your responses will be reviewed, as we are working on an expedited timeline to review responses and proceed with the distribution of LIHWAP funding

As the survey will include specific questions regarding the accompanying documents, we encourage you to review all documents before beginning the survey, and to have the documents at hand as you complete the survey for easy reference.
