NOTE: See the AFI PPR Overview page for additional information.

In 2016, OCS created two data tracking tools in Excel for AFI grantees needing a simple way to track project data that correspond with the AFI PPR. Use of these forms is not mandatory – many grantees have set up on their own tools or software to manage project data, but some grantees will find these Excel tools sufficient for their needs.

Please read through all instructions in the Welcome and How-To tabs within the tool. The tool utilizes formulas, data validation, and locked cells in order to protect data integrity. Using the form in unintended ways may cause unexpected results. If the instructions are not followed, these features may not work properly and your cumulative data may be incorrect.

  • Participant Version of Excel Data Tracking Tool for AFI PPR - The Participant Version of the Excel Tracking Tool (available for download below) is set up so information for each participant can be recorded. Formulas in the tool will then do the work of rolling-up cumulative numbers.
  • Partner Version of Excel Data Tracking Tool for AFI PPR - The Partner Version of the Excel Tracking Tool (available for download below) is meant to be used by AFI projects that have partners which independently open and administer AFI IDAs. The grantee organization will have each partner organization collect their own participant data in the Participant version, and then can copy partners’ participant data into the Partner version which will roll-up cumulative numbers for the project.
