Introduction

This is the seventh of nine modules in the Continuous Quality Improvement Toolkit: A Resource for Maternal, Infant, and Early Childhood Home Visiting Program Awardees (CQI Toolkit). The toolkit supports MIECHV awardees and others in working with local agencies to build capacity in CQI. Modules may be delivered individually by CQI staff or as part of a multiday training activity.

Module 7 helps participants—

  • Describe root cause analysis and three root cause analysis tools
  • Apply systematic processes to identify root causes
  • Analyze findings to determine where to focus CQI efforts

As with all modules in the CQI Toolkit, Module 7 contains a facilitation guide, PowerPoint slides with facilitation notes, and handouts and activities for participants. For example, Module 7 includes activities applying the “5 Whys” and “Fishbone Diagram” methods for identifying the root cause of an identified problem.
