The parent measure short form (English version) is intended for parents of children birth through 5 years old who are cared for by providers or teachers in a center-based, family child care, or Head Start/Early Head Start program. This English version of the short form asks parents questions about how they work with their child’s lead provider or teacher (not aides or assistant teachers), such as how easy or difficult it is to reach their provider/teacher during the day and how comfortable they feel talking to their provider/teacher about various topics.  It includes 25 items (including 8 demographic items) and takes about 5 minutes to complete, on average.

The Excel scoring sheet for the English version of the parent measure short form can be used to calculate overall, construct, and subscale scores in the parent measure short form.  Additional information on scoring is available in Chapter 5 of the Family and Provider/Teacher Relationship Quality Measures: User’s Manual.

The parent measure short form (Spanish version) is intended for parents of children birth through 5 years old who are cared for by providers or teachers in a center-based, family child care, or Head Start/Early Head Start program. The Excel scoring sheet for the parent measure short form (revised April 2015) can be used to automatically calculate overall, construct, and subscale scores. It is available in English and Spanish. The English and Spanish versions of the Excel scoring sheet are exactly the same, with the exception of the language of the question stem. Both English and Spanish versions of the parent measure short form can be entered into the same Excel scoring sheet for the parent measure short form.
