SF-424 Application for Federal Assistance

Implementation of the Federal Financial Reporting Form (SF-425)

All affected ACF grant recipients are required to submit an SF-425 report as frequently as is required in the terms and conditions of their award using due dates for reports to PMS.

Fillable versions of the SF-425 form  (PDF) in Adobe PDF and MS-Excel formats (along with instructions)

Please note: ORR Cash and Medical Assistance grant recipients must submit the ORR-2 in lieu of the SF-425.
